Prior to your appointment you can prepare some of the necessary forms by filling them out at home and bringing them with you to your appointment.
Medical billing and insurance plans can be confusing. It is important that you understand your individual health plan and what it covers, including deductibles, coinsurance, and copays. Call the customer service number on the back of your insurance card or contact your employer’s benefits department to verify your coverage and for any questions regarding your health insurance plan.
